MLH1 (HGNC:7127): (mutL homolog 1) The protein encoded by this gene can heterodimerize with mismatch repair endonuclease PMS2 to form MutL alpha, part of the DNA mismatch repair system. When MutL alpha is bound by MutS beta and some accessory proteins, the PMS2 subunit of MutL alpha introduces a single-strand break near DNA mismatches, providing an entry point for exonuclease degradation. The encoded protein is also involved in DNA damage signaling and can heterodimerize with DNA mismatch repair protein MLH3 to form MutL gamma, which is involved in meiosis. The MLH1 c.125C>T (p.Ala42Val) variant has been reported in the published literature in an individual with ovarian cancer, however, tumor testing indicated no microsatellite instability or loss of MLH1 expression (PMID: 22006311 (2011)). Functional studies showed inconclusive results regarding the variant's impact on protein function (PMID: 15475387 (2004)). The frequency of this variant in the general population, 0.000004 (1/251434 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is damaging. Two additional published in silico models predict the variant to be pathogenic (PMIDs: 25871441 (2013), 26333163 (2015)). Based on the available information, we are unable to determine the clinical significance of this variant.

This missense variant replaces alanine with valine at codon 42 of the MLH1 protein. Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). Functional studies are conflicting in regard to the variant's impact on mismatch repair activity (PMID: 15475387, 36054288). This variant has been reported in an individuals affected with Lynch syndrome-associated disorders with tumor data showing normal mismatch repair protein expression via immunohistochemistry (PMID: 22006311; ClinVar SCV000216707.8). This variant has been identified in 1/251434 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.

The p.A42V variant (also known as c.125C>T), located in coding exon 2 of the MLH1 gene, results from a C to T substitution at nucleotide position 125. The alanine at codon 42 is replaced by valine, an amino acid with similar properties. This variant has been identified in a proband whose primary ovarian, peritoneal, or fallopian tube carcinoma showed low-level microsatellite instability and demonstrated normal mismatch repair protein expression by immunohistochemistry (IHC) (Walsh T et al. Proc. Natl. Acad. Sci. U.S.A. 2011 Nov;108(44):18032-7). This variant has also been identified in probands whose Lynch syndrome-associated tumors demonstrated normal mismatch repair protein expression by IHC (Ambry internal data). In multiple assays testing MLH1 function, this variant showed functionally normal and functionally indeterminant results (Ellison AR et al. Nucleic Acids Res. 2004 Oct;32(18):5321-38; Rath A et al. Hum Mutat, 2022 Dec;43:2295-2307). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear.

Variant summary: MLH1 c.125C>T (p.Ala42Val) results in a non-conservative amino acid change located in the DNA mismatch repair protein family, N-terminal domain (IPR002099) of the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4e-06 in 251434 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.125C>T has been reported in the literature in at least one individual with ovarian cancer. Tumor analysis from this individual showed low microsatellite instability (Walsh_2011). This report does not provide unequivocal conclusions about association of the variant with Lynch Syndrome. A functional assay using a hybrid yeast screen showed the variant had >67% loss-of-MMR function (Ellison_2004). Seven ClinVar submitters have assessed the variant since 2014: all classified the variant as of uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance.
