Genes affected
MLH1 (HGNC:7127): (mutL homolog 1) The protein encoded by this gene can heterodimerize with mismatch repair endonuclease PMS2 to form MutL alpha, part of the DNA mismatch repair system. When MutL alpha is bound by MutS beta and some accessory proteins, the PMS2 subunit of MutL alpha introduces a single-strand break near DNA mismatches, providing an entry point for exonuclease degradation. The encoded protein is also involved in DNA damage signaling and can heterodimerize with DNA mismatch repair protein MLH3 to form MutL gamma, which is involved in meiosis. This gene was identified as a locus frequently mutated in hereditary nonpolyposis colon cancer (HNPCC). [provided by RefSeq, Aug 2017]

ACMG classification

Classification made for transcript

Verdict is Pathogenic. Variant got 16 ACMG points.

PVS1
Splicing variant, NOT destroyed by nmd, known LOF gene, truncates exone, which is 0.04095112 fraction of the gene. Cryptic splice site detected, with MaxEntScore 5.9, offset of 12, new splice context is: ttgtttacatcgtctggtAGaat. Cryptic site results in inframe change. If cryptic site found is not functional and variant results in exon loss, it results in frameshift change.
PM2
Very rare variant in population databases, with high coverage;
PP3
Splicing scoreres supports a deletorius effect: Scorers claiming Pathogenic: dbscSNV1_ADA, dbscSNV1_RF. No scorers claiming Uncertain. Scorers claiming Benign: max_spliceai.
PP5
Variant 3-37017505-G-C is Pathogenic according to our data. Variant chr3-37017505-G-C is described in ClinVar as [Pathogenic]. Clinvar id is 90369.Status of the report is reviewed_by_expert_panel, 3 stars. Variant chr3-37017505-G-C is described in Lovd as [Pathogenic]. Variant chr3-37017505-G-C is described in Lovd as [Likely_pathogenic].

Pathogenic, criteria provided, single submitterclinical testingInvitaeMar 14, 2020For these reasons, this variant has been classified as Pathogenic. Donor and acceptor splice site variants typically lead to a loss of protein function (PMID: 16199547), and loss-of-function variants in MLH1 are known to be pathogenic (PMID: 15713769, 24362816). Based on a multifactorial likelihood algorithm using genetic, tumor, and functional data, this variant has been determined to have a high probability of being pathogenic (PMID: 22949379). This variant has been observed in individual(s) with clinical features of Lynch syndrome (PMID: 8571956). ClinVar contains an entry for this variant (Variation ID: 90369). This variant is not present in population databases (ExAC no frequency). This sequence change affects an acceptor splice site in intron 9 of the MLH1 gene. It is expected to disrupt RNA splicing and likely results in an absent or disrupted protein product. -
Hereditary cancer-predisposing syndrome Pathogenic:1
Pathogenic, criteria provided, single submitterclinical testingColor Diagnostics, LLC DBA Color HealthMar 15, 2022This variant causes a G to C nucleotide substitution at the -1 position of intron 9 of the MLH1 gene. Functional RNA studies have shown that this variant causes exon 10 skipping, resulting in premature truncation (PMID: 22949379, 26247049, 29706640). This variant has been reported in individuals affected with Lynch syndrome (PMID: 8571956, 2414824; DOI: 10.3343/lmo.2018.8.4.156). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). Loss of MLH1 function is a known mechanism of disease (clinicalgenome.org). Based on the available evidence, this variant is classified as Pathogenic. -
