Variant summary
Our verdict is Likely benign. The variant received -2 ACMG points: 2P and 4B. PM2BP4_ModerateBP6BP7
The NM_000249.4(MLH1):c.882C>G(p.Leu294Leu) variant causes a splice region, synonymous change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.00000274 in 1,460,238 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. 3/3 splice prediction tools predict no significant impact on normal splicing. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Synonymous variant affecting the same amino acid position (i.e. L294L) has been classified as Pathogenic.

Hereditary cancer-predisposing syndrome Uncertain:1Benign:1
This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity.
This synonymous variant causes a C>G change in exon 10 of the MLH1 gene. Splice site prediction tools suggest that this variant may not impact RNA splicing. However, partial exon 10 skipping has been observed via RT-PCR analysis and a minigene assay (PMID: 26761715; doi: 10.1155/2024/4830045). This variant has not been reported in individuals affected with MLH1-related disorders in the literature. This variant has been identified in 1/251436 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.
Lynch syndrome Uncertain:1
This synonymous variant causes a C>G change in exon 10 of the MLH1 gene. Splice site prediction tools suggest that this variant may not impact RNA splicing. This variant has not been reported in individuals affected with MLH1-related disorders in the literature. The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.
Hereditary nonpolyposis colorectal neoplasms Uncertain:1
This variant has been reported in individual(s) in the Leiden Open-source Variation Database (PMID: 21520333). ClinVar contains an entry for this variant (Variation ID: 90406). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Studies have shown that this variant is associated with altered splicing, but the impact on the resulting protein product is unknown (PMID: 26761715). This variant is present in population databases (rs63751707, ExAC 0.001%). This sequence change affects codon 294 of the MLH1 mRNA. It is a 'silent' change, meaning that it does not change the encoded amino acid sequence of the MLH1 protein.
