This variant is present in population databases (rs63750443, gnomAD 0.003%). This missense change has been observed in individual(s) with gastric cancer (PMID: 15099349). ClinVar contains an entry for this variant (Variation ID: 89693). Invitae Evidence Modeling incorporating data from in vitro experimental studies (internal data) indicates that this missense variant is not expected to disrupt MLH1 function with a negative predictive value of 95%. Experimental studies have shown that this missense change does not substantially affect MLH1 function (PMID: 21136174). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

The p.E433Q variant (also known as c.1297G>C), located in coding exon 12 of the MLH1 gene, results from a G to C substitution at nucleotide position 1297. The glutamic acid at codon 433 is replaced by glutamine, an amino acid with highly similar properties. This variant was identified in individual(s) diagnosed with gastric cancer with microsatellite instability (Hudler P et al. Clin Genet, 2004 May;65:405-11). This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear. -
