MLH1 (HGNC:7127): (mutL homolog 1) The protein encoded by this gene can heterodimerize with mismatch repair endonuclease PMS2 to form MutL alpha, part of the DNA mismatch repair system. When MutL alpha is bound by MutS beta and some accessory proteins, the PMS2 subunit of MutL alpha introduces a single-strand break near DNA mismatches, providing an entry point for exonuclease degradation. The encoded protein is also involved in DNA damage signaling and can heterodimerize with DNA mismatch repair protein MLH3 to form MutL gamma, which is involved in meiosis. The p.D450Y variant (also known as c.1348G>T), located in coding exon 12 of the MLH1 gene, results from a G to T substitution at nucleotide position 1348. The aspartic acid at codon 450 is replaced by tyrosine, an amino acid with highly dissimilar properties. This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear. -

The missense variant NM_000249.4(MLH1):c.1348G>T (p.Asp450Tyr) has not been reported previously as a pathogenic variant nor as a benign variant, to our knowledge. The p.Asp450Tyr variant is observed in 1/18,392 (0.0054%) alleles from individuals of gnomAD East Asian background in gnomAD. The p.Asp450Tyr variant is novel (not in any individuals) in 1kG. There is a large physicochemical difference between aspartic acid and tyrosine, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. The gene MLH1 has a low rate of benign missense variation as indicated by a high missense variants Z-Score of 1.08. The gene MLH1 contains 237 pathogenic missense variants, indicating that missense variants are a common mechanism of disease in this gene. For these reasons, this variant has been classified as Uncertain Significance. -

The MLH1 c.1348G>T variant is predicted to result in the amino acid substitution p.Asp450Tyr. This variant was reported as a variant of uncertain significance in an individual with pancreatic ductal adenocarcinoma with no family history of cancer (eTable 4. Yin et al. 2022. PubMed ID: 35171259) and also in a large study of biliary tract cancers (Table S2. Okawa et al. 2022. PubMed ID: 36243179).
