Genes affected
MLH1 (HGNC:7127): (mutL homolog 1) The protein encoded by this gene can heterodimerize with mismatch repair endonuclease PMS2 to form MutL alpha, part of the DNA mismatch repair system. When MutL alpha is bound by MutS beta and some accessory proteins, the PMS2 subunit of MutL alpha introduces a single-strand break near DNA mismatches, providing an entry point for exonuclease degradation. The encoded protein is also involved in DNA damage signaling and can heterodimerize with DNA mismatch repair protein MLH3 to form MutL gamma, which is involved in meiosis. This gene was identified as a locus frequently mutated in hereditary nonpolyposis colon cancer (HNPCC). [provided by RefSeq, Aug 2017]

ACMG classification

Classification was made for transcript

Our verdict: Pathogenic. The variant received 18 ACMG points.

PVS1
Splicing +-2 bp (donor or acceptor) variant, LoF is a know mechanism of disease, Cryptic splice site detected, with MaxEntScore 3.9, offset of 5, new splice context is: catttttattttcactgaAGaac. Cryptic site results in frameshift change. If cryptic site found is not functional and variant results in exon loss, it results in frameshift change.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 3-37042267-G-C is Pathogenic according to our data. Variant chr3-37042267-G-C is described in ClinVar as Pathogenic. ClinVar VariationId is 1777597.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.

This variant is considered pathogenic. This variant occurs within a consensus splice junction and is predicted to result in abnormal mRNA splicing of either an out-of-frame exon or an in-frame exon necessary for protein stability and/or normal function. This variant is strongly associated with more severe personal and family histories of cancer, typical for individuals with pathogenic variants in this gene [PMID: 27363726].

The c.1668-1G>C pathogenic variant results from a G to C substitution one nucleotide upstream from coding exon 15 of the MLH1 gene. This variant was identified in an individual who met Amsterdam II criteria for Lynch syndrome and colorectal tumor demonstrated high microsatellite instability (MSI-H) with loss of PMS2 expression on immunohistochemistry (IHC) (Ambry internal data). This variant has also been reported in individuals suspected of having Lynch syndrome and one proband had a colorectal tumor that demonstrated loss of MLH1 protein expression on IHC (Kang SY et al. Int. J. Cancer, 2015 Apr;136:1568-78; Park KJ et al. Lab Med Online, 2018 Oct;8,4:156-166; https://doi.org/10.3343/lmo.2018.8.4.156). In addition to the clinical data presented in the literature, alterations that disrupt the canonical splice site are expected to cause aberrant splicing, resulting in an abnormal protein or a transcript that is subject to nonsense-mediated mRNA decay. As such, this alteration is classified as a disease-causing mutation.
