Variant summary
Our verdict is Likely pathogenic. Variant got 6 ACMG points: 6P and 0B. PM1PM2PP3_Moderate
The NM_000249.4(MLH1):āc.1760T>Cā(p.Met587Thr) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.00000657 in 152,194 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (ā ā ). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. M587I) has been classified as Uncertain significance.

not provided Uncertain:1
Uncertain significance, criteria provided, single submitter | clinical testing | GeneDx | Apr 24, 2019 | Not observed in large population cohorts (Lek et al., 2016); Has not been previously published as pathogenic or benign to our knowledge - |
Hereditary nonpolyposis colorectal neoplasms Uncertain:1
Uncertain significance, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Apr 05, 2018 | This variant is not present in population databases (ExAC no frequency). This sequence change replaces methionine with threonine at codon 587 of the MLH1 protein (p.Met587Thr). The methionine residue is highly conserved and there is a moderate physicochemical difference between methionine and threonine. This variant has been reported in an individual in the InSiGHT database (https://www.insight-database.org/individuals/00016356). However, in that individual, a pathogenic MLH1 variant was also identified, which suggests that this c.1760T>C variant was not the primary cause of disease. ClinVar contains an entry for this variant (Variation ID: 89881). Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. - |
Hereditary cancer-predisposing syndrome Uncertain:1
Uncertain significance, criteria provided, single submitter | clinical testing | Ambry Genetics | Nov 15, 2022 | The p.M587T variant (also known as c.1760T>C), located in coding exon 16 of the MLH1 gene, results from a T to C substitution at nucleotide position 1760. The methionine at codon 587 is replaced by threonine, an amino acid with similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. - |
