This variant changes 1 nucleotide in exon 16 of the MLH1 gene, creating a premature translation stop signal. This variant is expected to result in an absent or non-functional protein product. This variant has been reported in an individual affected with Lynch syndrome (PMID: 27064304). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). Loss of MLH1 function is a known mechanism of disease (clinicalgenome.org). Based on the available evidence, this variant is classified as Pathogenic.
The p.L590* pathogenic mutation (also known as c.1769T>G), located in coding exon 16 of the MLH1 gene, results from a T to G substitution at nucleotide position 1769. This changes the amino acid from a leucine to a stop codon within coding exon 16. This mutation was reported in a French family with suspected HNPCC/Lynch syndrome (Parc Y et al. J Med Genet. 2003 Mar;40:208-13). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation.

The variant was absent in 251254 control chromosomes (gnomAD). c.1769T>G has been reported in the literature in individuals/families affected with Hereditary Nonpolyposis Colorectal Cancer (Parc_2003, Sjursen_2016). These data indicate that the variant may be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 12624141, 27064304). Three ClinVar submitters have assessed the variant since 2014, and all three classified the variant as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic.
