Colorectal cancer, hereditary nonpolyposis, type 2 Uncertain:2Benign:1
This variant is considered benign. This variant is strongly associated with less severe personal and family histories of cancer, typical for individuals without pathogenic variants in this gene [PMID: 27363726]. Homozygosity for this variant has been confirmed in one or more individuals lacking clinical features consistent with gene-specific recessive disease, indicating that this variant is unlikely to be pathogenic.
Hereditary cancer-predisposing syndrome Uncertain:2
This missense variant replaces proline with serine at codon 747 of the MLH1 protein. Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in an individual affected with early-onset colorectal cancer (PMID: 31360874). This variant has been identified in 1/251292 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.
The p.P747S variant (also known as c.2239C>T), located in coding exon 19 of the MLH1 gene, results from a C to T substitution at nucleotide position 2239. The proline at codon 747 is replaced by serine, an amino acid with similar properties. This variant was identified in an individual diagnosed with colorectal cancer (Toh MR et al. JNCI Cancer Spectr, 2018 Oct;2:pky054). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear.
not specified Uncertain:1
Variant summary: MLH1 c.2239C>T (p.Pro747Ser) results in a non-conservative amino acid change located in the DNA mismatch repair protein Mlh1 C-terminus (IPR032189) of the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4e-06 in 251292 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.2239C>T has been reported in the literature in individuals with breast or colorectal cancer undergoing multigene panel testing (e.g. Yorczyk_2015, Toh_2018, Pearlman_2021) These report(s) do not provide unequivocal conclusions about association of the variant with Hereditary Nonpolyposis Colorectal Cancer. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 34250417, 31360874, 25318351). ClinVar contains an entry for this variant (Variation ID: 127622). Based on the evidence outlined above, the variant was classified as uncertain significance.

Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; This variant is associated with the following publications: (PMID: 12799449, 20533529, 22753075, 31360874, 25318351)
