BAP1-related tumor predisposition syndrome Uncertain:2
This sequence change replaces isoleucine, which is neutral and non-polar, with threonine, which is neutral and polar, at codon 643 of the BAP1 protein (p.Ile643Thr). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with multiple primary cutaneous melanomas (PMID: 31382929). ClinVar contains an entry for this variant (Variation ID: 490825). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt BAP1 protein function with a positive predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
Hereditary cancer-predisposing syndrome Uncertain:1Benign:1
This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity.
This missense variant replaces isoleucine with threonine at codon 643 of the BAP1 protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in individuals affected with multiple primary melanomas (PMID: 31382929). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.
not provided Uncertain:1
Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; Observed in individuals with cutaneous malignant melanoma; however, many individuals in the study were reported to harbor other pathogenic variants (PMID: 31382929); This variant is associated with the following publications: (PMID: 32028647, 31382929)
BAP1-related disorder Uncertain:1
The BAP1 c.1928T>C variant is predicted to result in the amino acid substitution p.Ile643Thr. This variant was reported in twenty one individuals with multiple primary melanomas, however all those patients were located in the same geographic region (southern Italy) and some patients carried also other potentially causative variants (Casula et al. 2019. PubMed ID: 31382929). This variant has not been reported in a large population database , indicating this variant is rare. At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence.
