BAP1 (HGNC:950): (BRCA1 associated protein 1) This gene belongs to the ubiquitin C-terminal hydrolase subfamily of deubiquitinating enzymes that are involved in the removal of ubiquitin from proteins. The encoded enzyme binds to the breast cancer type 1 susceptibility protein (BRCA1) via the RING finger domain of the latter and acts as a tumor suppressor. In addition, the enzyme may be involved in regulation of transcription, regulation of cell cycle and growth, response to DNA damage and chromatin dynamics. Germline mutations in this gene may be associated with tumor predisposition syndrome (TPDS), which involves increased risk of cancers including malignant mesothelioma, uveal melanoma and cutaneous melanoma. The BAP1 c.1441C>A (p.His481Asn) variant has been reported in the published literature in individuals with breast and/or ovarian cancer (PMID: 30306255 (2018)) and peritoneal mesothelioma (PMID: 28034829 (2017)). This variant was also identified in an individual with cutaneous and uveal melanoma (PMID: 30517737 (2018)). The frequency of this variant in the general population, 0.000079 (4/50756 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. Based on the available information, we are unable to determine the clinical significance of this variant.

This sequence change replaces histidine, which is basic and polar, with asparagine, which is neutral and polar, at codon 481 of the BAP1 protein (p.His481Asn). This variant is present in population databases (rs764816701, gnomAD 0.006%). This missense change has been observed in individual(s) with malignant pleural mesothelioma (MPM), which was found in a tumor sample but not confirmed as a germline variant (PMID: 28034829). ClinVar contains an entry for this variant (Variation ID: 472669). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt BAP1 protein function with a negative predictive value of 80%.
