Genes affected
AGA (HGNC:318): (aspartylglucosaminidase) This gene encodes a member of the N-terminal nucleophile (Ntn) hydrolase family of proteins. The encoded preproprotein is proteolytically processed to generate alpha and beta chains that comprise the mature enzyme. This enzyme is involved in the catabolism of N-linked oligosaccharides of glycoproteins. It cleaves asparagine from N-acetylglucosamines as one of the final steps in the lysosomal breakdown of glycoproteins. Mutations in this gene are associated with the lysosomal storage disease aspartylglycosaminuria that results in progressive neurodegeneration. Alternative splicing results in multiple transcript variants, at least one of which encodes an isoform that is subject to proteolytic processing. [provided by RefSeq, Nov 2015]

ACMG classification

Classification made for transcript

Verdict is Pathogenic. Variant got 18 ACMG points.

PVS1
Splicing +-2 bp (donor or acceptor) variant, LoF is a know mechanism of disease, Cryptic splice site detected, with MaxEntScore 3.2, offset of -16, new splice context is: aatGTgact. Cryptic site results in frameshift change. If cryptic site found is not functional and variant results in exon loss, it results in frameshift change.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 4-177433213-C-T is Pathogenic according to our data. Variant chr4-177433213-C-T is described in ClinVar as [Likely_pathogenic]. Clinvar id is 555229.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.

Aspartylglucosaminuria Pathogenic:2
Pathogenic, criteria provided, single submitterclinical testingLabcorp Genetics (formerly Invitae), LabcorpApr 03, 2023ClinVar contains an entry for this variant (Variation ID: 555229). Disruption of this splice site has been observed in individual(s) with aspartylglucosaminuria (PMID: 1879549, 6883788). This variant is not present in population databases (gnomAD no frequency). This sequence change affects a donor splice site in intron 8 of the AGA gene. RNA analysis indicates that disruption of this splice site induces altered splicing and likely disrupts the C-terminus of the protein. Algorithms developed to predict the effect of variants on protein structure and function are not available or were not evaluated for this variant. For these reasons, this variant has been classified as Pathogenic. Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Studies have shown that disruption of this splice site results in skipping of exon 8 and introduces a new termination codon (PMID: 1879549). However the mRNA is not expected to undergo nonsense-mediated decay. Experimental studies have shown that disruption of this splice site affects AGA function (PMID: 1879549). -
