Genes affected
FGFR3 (HGNC:3690): (fibroblast growth factor receptor 3) This gene encodes a member of the fibroblast growth factor receptor (FGFR) family, with its amino acid sequence being highly conserved between members and among divergent species. FGFR family members differ from one another in their ligand affinities and tissue distribution. A full-length representative protein would consist of an extracellular region, composed of three immunoglobulin-like domains, a single hydrophobic membrane-spanning segment and a cytoplasmic tyrosine kinase domain. The extracellular portion of the protein interacts with fibroblast growth factors, setting in motion a cascade of downstream signals, ultimately influencing mitogenesis and differentiation. This particular family member binds acidic and basic fibroblast growth hormone and plays a role in bone development and maintenance. Mutations in this gene lead to craniosynostosis and multiple types of skeletal dysplasia. [provided by RefSeq, Aug 2017]

ACMG classification

Classification made for transcript

Verdict is Likely_pathogenic. Variant got 8 ACMG points.

PVS1
Loss of function variant, product does not undergo nonsense mediated mRNA decay. Variant is located in the 3'-most exon, not predicted to undergo nonsense mediated mRNA decay. There are 5 pathogenic variants in the truncated region.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 4-1807262-A-T is Pathogenic according to our data. Variant chr4-1807262-A-T is described in ClinVar as [Pathogenic]. Clinvar id is 16336.Status of the report is criteria_provided_single_submitter, 1 stars. Variant chr4-1807262-A-T is described in Lovd as [Pathogenic].

Pathogenic, criteria provided, single submitterclinical testingPreventionGenetics, part of Exact SciencesSep 09, 2023The FGFR3 c.2421A>T variant is predicted to result in extension of the open reading frame (p.*807Cysext*101). This variant was reported in individuals with thanatophoric dysplasia (Patient 2 in Rousseau et al. 1995. PubMed ID: 7647778; Soo-Kyeong et al. 2018. PubMed ID: 30252581). This variant has not been reported in a large population database (http://gnomad.broadinstitute.org), indicating this variant is rare. In ClinVar, this variant has been interpreted as pathogenic. In addition, other nucleotide changes at c.2421 (c.241A>C and c.241A>G), resulting in a stop loss and protein extension, have been reported in patients with thanatophoric dysplasia (Passos-Bueno et al. 1999. PubMed ID: 10425034).This variant is interpreted as pathogenic. -
