Variant summary
Our verdict is Pathogenic. The variant received 14 ACMG points: 14P and 0B. PM2PM5PP3_ModeratePP5_Very_Strong
The NM_173660.5(DOK7):c.437C>T(p.Pro146Leu) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.0000205 in 1,613,668 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Likely pathogenic (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. P146S) has been classified as Likely pathogenic.

Fetal akinesia deformation sequence 1;C1850792:Congenital myasthenic syndrome 10 Pathogenic:1
This sequence change replaces proline, which is neutral and non-polar, with leucine, which is neutral and non-polar, at codon 146 of the DOK7 protein (p.Pro146Leu). This variant is present in population databases (rs770987150, gnomAD 0.02%). This missense change has been observed in individuals with congenital myasthenic syndrome (PMID: 20012313, 30266093; internal data). ClinVar contains an entry for this variant (Variation ID: 560990). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt DOK7 protein function with a positive predictive value of 80%. For these reasons, this variant has been classified as Pathogenic. -

Fetal akinesia deformation sequence 1 Pathogenic:1
This mutation has been previously reported as disease-causing and was found once in our laboratory in trans with another pathogenic mutation in a fetal demise with fetal dyskinesia. -
Congenital myasthenic syndrome Pathogenic:1
Variant summary: DOK7 c.437C>T (p.Pro146Leu) results in a non-conservative amino acid change located in the Pleckstrin-homology domain (PH domain)/Phosphotyrosine-binding domain (PTB) (IPR011993) of the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 2.4e-05 in 250728 control chromosomes. c.437C>T has been reported in the literature in multiple compound heterozygous individuals affected with Congenital Myasthenic Syndrome or with fetal structural anomalies consistent with this phenotype (e.g. Ammar_2010, Normand_2018, Polavapu_2024, Labcorp (formerly Invitae)) . These data indicate that the variant is very likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 20012313, 30266093, 37721175). ClinVar contains an entry for this variant (Variation ID: 560990). Based on the evidence outlined above, the variant was classified as pathogenic. -
