Familial adenomatous polyposis 1 Uncertain:4
This sequence change replaces serine, which is neutral and polar, with arginine, which is basic and polar, at codon 306 of the APC protein (p.Ser306Arg). This variant is present in population databases (rs730881231, gnomAD 0.005%). This variant has not been reported in the literature in individuals affected with APC-related conditions. ClinVar contains an entry for this variant (Variation ID: 181783). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt APC protein function with a negative predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
This submission and the accompanying classification are no longer maintained by the submitter. For more information on current observations and classification, please contact variantquestions@myriad.com.
This variant is classified as a variant of uncertain significance as there is insufficient evidence to determine its impact on protein function and/or cancer risk.
not specified Uncertain:2
Variant summary: APC c.918T>G (p.Ser306Arg) results in a non-conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 2.4e-05 in 251136 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.918T>G in individuals affected with Familial Adenomatous Polyposis and no experimental evidence demonstrating its impact on protein function have been reported. Four clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. All laboratories classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance.
Hereditary cancer-predisposing syndrome Benign:2
This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity.
not provided Uncertain:1
In silico analysis supports that this missense variant does not alter protein structure/function; Observed in an individual with melanoma (Yehia et al., 2018); This variant is associated with the following publications: (PMID: 29684080)
Classic or attenuated familial adenomatous polyposis Uncertain:1
This missense variant replaces serine with arginine at codon 306 of the APC protein. Computational prediction is inconclusive regarding the impact of this variant on protein structure and function (internally defined REVEL score threshold 0.5 < inconclusive < 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has not been reported in individuals affected with APC-related disorders in the literature. This variant has been identified in 6/251136 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.
Gardner syndrome;C5679615:APC-related attenuated familial adenomatous polyposis Uncertain:1
APC-related disorder Uncertain:1
The APC c.918T>G variant is predicted to result in the amino acid substitution p.Ser306Arg. This variant, along with another missense change in the same gene, has been reported in a cohort study of individuals with Cowden syndrome and Bannayan-Riley-Ruvalcaba syndrome (S9 Table, Yehia et al 2018. PubMed ID: 29684080). This variant is reported in 0.0053% of alleles in individuals of European (Non-Finnish) descent in gnomAD. This variant is interpreted as a variant of uncertain significance by majority of the submitters in ClinVar (https://www.ncbi.nlm.nih.gov/clinvar/variation/181783/). At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence.
