For these reasons, this variant has been classified as Pathogenic. Studies have shown that disruption of this splice site results in skipping of exon 12 (also known as exon 11) and introduces a premature termination codon (PMID: 22987206; Invitae). The resulting mRNA is expected to undergo nonsense-mediated decay. This sequence change affects a donor splice site in intron 12 of the APC gene. RNA analysis indicates that disruption of this splice site induces altered splicing and may result in an absent or disrupted protein product. This variant is not present in population databases (gnomAD no frequency). ClinVar contains an entry for this variant (Variation ID: 428160). This variant is also known as IVS11+1G>T. Disruption of this splice site has been observed in individual(s) with clinical features of APC-related conditions (PMID: 11933206). It has also been observed to segregate with disease in related individuals. -

The c.1548+1G>T intronic pathogenic mutation results from a G to T substitution one nucleotide after coding exon 11 of the APC gene. This mutation has been reported in an FAP family from the Czech Republic and functional studies have shown that it leads to aberrant splicing with two separate truncated protein transcripts observed: one with skipping of exon 11 and one with included both exon 11 and intron 11 (Schwarzová L et al. Fam. Cancer 2013 Mar; 12(1):35-42; Kohoutová M et al. Hum. Mutat. 2002 Apr; 19(4):460-1). Of note, this alteration is also referred to as IVS11+1G>T in published literature. In addition to the clinical data presented in the literature, alterations that disrupt the canonical splice site are expected to cause aberrant splicing, resulting in an abnormal protein or a transcript that is subject to nonsense-mediated mRNA decay. As such, this alteration is classified as pathogenic. -
