PM2_Supporting, BP1 c.2819C>T, located in exon 16 of the APC gene, is predicted to result in the substitution of Ser by Leu at codon 940, p.(Ser940Leu) (BP1).It is not present in the population database gnomAD v2.1.1, non cancer dataset (PM2_supporting). The SpliceAI algorithm predicts no significant impact on splicing. To our knowledge, functional studies have not been reported for this variant. At present ClinVar does not describe pathogenic or likely pathogenic missense variants in this codon. This variant was identified in a patient affected with colorectal cancer (internal data). This variant has been reported in the ClinVar database (8x uncertain significance) but it has not been reported in LOVD database. Based on currently available information, the variant c.2819C>T should be considered an uncertain significance variant. -
The p.S940L variant (also known as c.2819C>T), located in coding exon 15 of the APC gene, results from a C to T substitution at nucleotide position 2819. The serine at codon 940 is replaced by leucine, an amino acid with dissimilar properties. This amino acid position is not well conserved in available vertebrate species. In addition, in silico predictors for this gene do not accurately predict pathogenicity. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

The APC p.Ser940Leu variant was not identified in the literature nor was it identified in the 1000 Genomes Project, NHLBI GO Exome Sequencing Project, the Exome Aggregation Consortium (March 14, 2016), Clinvitae, COSMIC, InSiGHT Colon Cancer Gene Variant (LOVD), Zhejiang Colon Cancer (LOVD), ClinVar, GeneInsight – COGR and UMD databases. The variant was identified in dbSNP (ID: rs544709767) as “NA”, but no frequency information was provided; thus the prevalence of this variant in the general population could not be determined. The variant was identified in our lab co-occurring with a pathogenic variant (c.3786_3787delTT) in a case of classical FAP. The p.Ser940 residue is conserved in mammals and computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) provide inconsistent predictions regarding the impact to the protein; this information is not very predictive of pathogenicity. The variant occurs outside of the splicing consensus sequence and 1 of 5 in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer, HumanSpliceFinder) predict a greater than 10% difference in splicing; this is not very predictive of pathogenicity.
