It is also involved in other processes including cell migration and adhesion, transcriptional activation, and apoptosis. Defects in this gene cause familial adenomatous polyposis (FAP), an autosomal dominant pre-malignant disease that usually progresses to malignancy. Mutations in the APC gene have been found to occur in most colorectal cancers, where disease-associated mutations tend to be clustered in a small region designated the mutation cluster region (MCR) and result in a truncated protein product. Truncating variants in APC are known to be pathogenic. This particular truncation has been reported in an individual with familial adenomatous polyposis (FAP) (PMID: 9375853). In addition, multiple truncating variants downstream of this truncation have been reported as pathogenic in individuals with FAP (PMID: 17064931). This variant is also known as 3239delAG (T1082X) in the literature. This sequence change deletes 2 nucleotides from exon 16 of the APC mRNA (c.3241_3242delAG), causing a frameshift at codon 1081. This creates a premature translational stop signal in the last exon of the APC mRNA (p.Ser1081Hisfs*2). While this is not anticipated to result in nonsense mediated decay, it is expected to result in a truncated APC protein. -

This mutation (referred to as 3239delAG) has been reported in an individual from northwest England who was diagnosed with familial adenomatous polyposis (Armstrong JG et al. Hum. Mutat. 1997; 10(5):376-80). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation. As such, this alteration is interpreted as a disease-causing mutation. -
