Four of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 9.5e-05 in 285314 control chromosomes (gnomAD and publication data). The observed variant frequency is approximately 1.3 fold of the estimated maximal expected allele frequency for a pathogenic variant in APC causing Familial Adenomatous Polyposis phenotype (7.1e-05), strongly suggesting that the variant is benign. c.3875C>T has been reported in the literature as a VUS in individuals affected with Familial Adenomatous Polyposis (Nagase_1992, Kim_2019), Breast Cancer (Tung_2015) or CRC (Chubb_2015, Yurgelun_2017, Sekine_2017). Nagase_1992 in particular reported the variant within a group of variants that they determined did not cosegregate with FAP phenotype and another mutation that led to translational termination was detected. These reports do not provide unequivocal conclusions about association of the variant with Familial Adenomatous Polyposis. Co-occurrences with other pathogenic variants have been reported (ATM c.2467-1G>A; RNF43 c.1976delG, p.G659fs; APC c.3631_3632del, p.Met1211ValfsTer5) (Internal data, Sekine_2017, Kim_2019), providing supporting evidence for a benign role. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Eleven ClinVar submitters (evaluation after 2014) cite the variant as uncertain significance (n=7) and likely benign (n=4). Based on the evidence outlined above, the variant was classified as likely benign. - |
