This variant is classified as a variant of uncertain significance as there is insufficient evidence to determine its impact on protein function and/or cancer risk. -

This sequence change replaces proline, which is neutral and non-polar, with leucine, which is neutral and non-polar, at codon 1960 of the APC protein (p.Pro1960Leu). This variant is present in population databases (rs587781546, gnomAD 0.007%). This missense change has been observed in individual(s) with colorectal cancer (PMID: 27978560, 28135145). ClinVar contains an entry for this variant (Variation ID: 141168). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt APC protein function with a negative predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; This variant is associated with the following publications: (PMID: 26927868, 27978560, 28135145, 18199528) -
The APC p.Pro1970Leu variant was identified in 2 of 1958 proband chromosomes (frequency: 0.001) from individuals with colorectal cancer and was a classified as a VUS by these studies (Pearlman_2017_PMID 27978560; Yurgelun_2017_PMID 28135145). The variant was also identified in dbSNP (ID: rs587781546), ClinVar (classified as a VUS by Ambry Genetics and Counsyl for Familial adenomatous polyposis 1 and Hereditary cancer-predisposing syndrome), LOVD 3.0 (classified as a VUS) and UMD-LSDB but was not identified in Cosmic. The variant was also identified in control databases in 10 of 282224 chromosomes at a frequency of 0.000035 (Genome Aggregation Database Feb 27, 2017). The variant was observed in the following populations: Other in 1 of 7200 chromosomes (freq: 0.000139) and European (non-Finnish) in 9 of 128708 chromosomes (freq: 0.00007), while the variant was not observed in the African, Latino, Ashkenazi Jewish, East Asian, European (Finnish), and South Asian populations. The p.Pro1970 residue is conserved across mammals and other organisms, and four out of five computational analyses (PolyPhen-2, SIFT, BLOSUM, MutationTaster) suggest that the variant may impact the protein; however, this information is not predictive enough to assume pathogenicity. The variant occurs outside of the splicing consensus sequence and in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer) do not predict a difference in splicing. In summary, based on the above information the clinical significance of this variant cannot be determined with certainty at this time. This variant is classified as a variant of uncertain significance. -
Hereditary cancer-predisposing syndrome Uncertain:1Benign:1
This missense variant replaces proline with leucine at codon 1960 of the APC protein. Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in individuals affected with colorectal cancer (PMID: 27978560, 28135145). This variant has been identified in 10/282224 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity. -
APC-Associated Polyposis Disorders Uncertain:1
This variant was observed as part of a predisposition screen in an ostensibly healthy population. A literature search was performed for the gene, cDNA change, and amino acid change (where applicable). Publications were found based on this search. However, the evidence from the literature, in combination with allele frequency data from public databases where available, was not sufficient to rule this variant in or out of causing disease. Therefore, this variant is classified as a variant of unknown significance. -
