Variant summary

Our verdict is Uncertain significance. The variant received 1 ACMG points: 2P and 1B. PM2BP4

The NM_000038.6(APC):c.6830C>G(p.Ser2277Cys) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a benign outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. S2277F) has been classified as Uncertain significance.

Genes affected
APC (HGNC:583): (APC regulator of WNT signaling pathway) This gene encodes a tumor suppressor protein that acts as an antagonist of the Wnt signaling pathway. It is also involved in other processes including cell migration and adhesion, transcriptional activation, and apoptosis. Defects in this gene cause familial adenomatous polyposis (FAP), an autosomal dominant pre-malignant disease that usually progresses to malignancy. Mutations in the APC gene have been found to occur in most colorectal cancers, where disease-associated mutations tend to be clustered in a small region designated the mutation cluster region (MCR) and result in a truncated protein product. Significance:Uncertain significance
Review Status:no assertion criteria provided
Collection Method:clinical testing

The p.Ser2277Cys variant has not been previously identified in the literature or in the public databases. The Ser2277 residue is not conserved in mammals, but in silico computational analysis (PolyPhen and SIFT) suggest an impact on protein function, however this is not predictive enough to suggest pathogenicity. In summary, based on the current information presented above, this variant is classified as VUS. -

Significance:Uncertain significance
Review Status:criteria provided, single submitter
Collection Method:clinical testing

The p.S2277C variant (also known as c.6830C>G), located in coding exon 15 of the APC gene, results from a C to G substitution at nucleotide position 6830. The serine at codon 2277 is replaced by cysteine, an amino acid with dissimilar properties. This amino acid position is not well conserved in available vertebrate species. In addition, in silico predictors for this gene do not accurately predict pathogenicity. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
