The APC p.Asp2656Tyr variant was identified in 1 of 1260 proband chromosomes (frequency: 0.001) from individuals or families with sproradic colorectal cancer (Christie 2013). The patient identified in this study was found to have a second APC variant c.3878delC. The patient tumour showed loss of heterozygosity. The variant was also identified in the COSMIC database. The variant was not identified in dbSNP, NHLBI Exome Sequencing Project (Exome Variant Server), the Exome Aggregation Consortium, Clinvitae database, InSiGHT Colon Cancer Gene Variant Database (LOVD), Zhejiang Colon Cancer Database (LOVD), ClinVar database, GeneInsight - COGR database, and UMD. The p.Asp2656 residue is conserved across mammals and other organisms, and computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) suggest that the variant may impact the protein; however, this information is not predictive enough to assume pathogenicity. The variant occurs outside of the splicing consensus sequence and 4 of 5 in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer, HumanSpliceFinder) predict a greater than 10% difference in splicing. The p.D2656Y variant (also known as c.7966G>T), located in coding exon 15 of the APC gene, results from a G to T substitution at nucleotide position 7966. The aspartic acid at codon 2656 is replaced by tyrosine, an amino acid with highly dissimilar properties. This amino acid position is conserved. In addition, in silico predictors for this gene do not accurately predict pathogenicity. Based on the available evidence, the clinical significance of this variant remains unclear. -
