Familial adenomatous polyposis 1 Uncertain:2
- -
This sequence change replaces glutamine, which is neutral and polar, with histidine, which is basic and polar, at codon 2727 of the APC protein (p.Gln2727His). This variant is present in population databases (no rsID available, gnomAD 0.0009%). This missense change has been observed in individual(s) with breast cancer (PMID: 30613976). ClinVar contains an entry for this variant (Variation ID: 505436). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt APC protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
not specified Uncertain:1
The p.Gln2727His variant in APC has not been previously reported in individuals with APC-associated cancer and was absent from large population studies. Computa tional prediction tools and conservation analysis do not provide strong support for or against an impact to the protein. In summary, the clinical significance o f the p.Gln2727His variant is uncertain. -
Classic or attenuated familial adenomatous polyposis Uncertain:1
This missense variant replaces glutamine with histidine at codon 2727 of the APC protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in an individual affected with colorectal cancer or polyps in the literature (PMID: 31422818). This variant has been identified in 1/251356 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
Hereditary cancer-predisposing syndrome Uncertain:1
The p.Q2727H variant (also known as c.8181G>C), located in coding exon 15 of the APC gene, results from a G to C substitution at nucleotide position 8181. The glutamine at codon 2727 is replaced by histidine, an amino acid with highly similar properties. This alteration was detected in 1 of 1165 individuals undergoing genetic testing based on a history of colorectal cancer or colon polyps (Gordon AS et al. Am J Hum Genet, 2019 09;105:526-533). This amino acid position is well conserved in available vertebrate species. In addition, in silico predictors for this gene do not accurately predict pathogenicity. Missense alterations in APC are not a common cause of disease (Spier I et al. Genet Med. 2024 Feb;26(2):100992). Based on the available evidence, the clinical significance of this variant remains unclear. -
