The p.R1200* pathogenic mutation (also known as c.3598C>T), located in coding exon 23 of the RAD50 gene, results from a C to T substitution at nucleotide position 3598. This changes the amino acid from an arginine to a stop codon within coding exon 23. This variant has been detected in 1/165 colorectal cancer and/or polyposis patients and not in 2512 control individuals from a healthy population database (Rosenthal EA et al. Hum Genet, 2018 Oct;137:795-806), in a glioblastoma multiforme cancer patient from a cohort of 4034 cancer cases from The Cancer Genome Atlas (Lu C et al. Nat Commun. 2015 Dec 22;6:10086), and in 1/1764 Chinese lung cancer patients (Tian P et al. Pathol Oncol Res, 2020 Jan;26:109-114). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation. -
This sequence change creates a premature translational stop signal (p.Arg1200*) in the RAD50 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in RAD50 are known to be pathogenic (PMID: 19409520). This variant is present in population databases (rs750586158, gnomAD 0.003%). This variant has not been reported in the literature in individuals affected with RAD50-related conditions. ClinVar contains an entry for this variant (Variation ID: 184879). For these reasons, this variant has been classified as Pathogenic. -

The RAD50 c.3598C>T (p.Arg1200*) variant is predicted to cause the premature termination of RAD50 protein synthesis. This variant has been reported in the published literature in individuals with breast cancer (PMID: 34371384 (2021), 38355628 (2024)), glioblastoma multiforme (PMID: 36451132 (2022), 29625052 (2018)), lung cancer (PMID: 31721094 (2018)), endometrial cancer (PMID: 29625052 (2018)), and colon cancer (PMID: 30267214 (2018)). This variant has also been observed in a reportedly healthy individual (PMID: 33471991 (2021), see also LOVD (http://databases.lovd.nl/shared)). The frequency of this variant in the general population, 0.000012 (3/251342 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Based on the available information, this variant is classified as likely pathogenic. -
