Mutations in this gene cause atrial septal defect with atrioventricular conduction defect, and also tetralogy of Fallot, which are both heart malformation diseases. Mutations in this gene can also cause congenital hypothyroidism non-goitrous type 5, a non-autoimmune condition. Alternative splicing results in multiple transcript variants. This variant is not present in population databases (gnomAD no frequency). This sequence change creates a premature translational stop signal (p.Ser146*) in the NKX2-5 gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 179 amino acid(s) of the NKX2-5 protein. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may create or strengthen a splice site. For these reasons, this variant has been classified as Pathogenic. This variant disrupts a region of the NKX2-5 protein in which other variant(s) (p.Ala262Argfs*32) have been determined to be pathogenic (PMID: 22920929). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. -

The p.S146* pathogenic mutation (also known as c.437C>A), located in coding exon 2 of the NKX2-5 gene, results from a C to A substitution at nucleotide position 437. This changes the amino acid from a serine to a stop codon within coding exon 2. This alteration occurs at the 3' terminus of theNKX2-5 gene, is not expected to trigger nonsense-mediated mRNA decay, and impacts the last 55% of the protein. However, premature stop codons are typically deleterious in nature, a significant portion of the protein is affected, and the impacted region is critical for protein function (Ambry internal data). This variant was identified in one or more individuals with features consistent with NKX2-5 related congenital heart disease, conduction disease, and cardiomyopathy, and segregated with disease in at least one family (Helm BM et al. Cardiol Young, 2024 Mar;34:654-658; Ambry internal data). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). Based on the supporting evidence, this variant is interpreted as a disease-causing mutation. -
