NSD1 (HGNC:14234): (nuclear receptor binding SET domain protein 1) This gene encodes a protein containing a SET domain, 2 LXXLL motifs, 3 nuclear translocation signals (NLSs), 4 plant homeodomain (PHD) finger regions, and a proline-rich region. The encoded protein enhances androgen receptor (AR) transactivation, and this enhancement can be increased further in the presence of other androgen receptor associated coregulators. This protein may act as a nucleus-localized, basic transcriptional factor and also as a bifunctional transcriptional regulator. Mutations of this gene have been associated with Sotos syndrome and Weaver syndrome. One version of childhood acute myeloid leukemia is the result of a cryptic translocation with the breakpoints occurring within nuclear receptor-binding Su-var, enhancer of zeste, and trithorax domain protein 1 on chromosome 5 and nucleoporin, 98-kd on chromosome 11. Functional study found R2005Q reduced the activity of the H3K36 HKMT Qiao et al., 2011. The p.Arg2005Gln variant is novel not in any individuals in gnomAD Exomes and 1000 Genomes. This variant has been reported to the ClinVar database as Likely Pathogenic/ Pathogenic. The amino acid change p.Arg2005Gln in NSD1 is predicted as conserved by GERP++ and PhyloP across 100 vertebrates. The amino acid Arg at position 2005 is changed to a Gln changing protein sequence and it might alter its composition and physico-chemical properties. For these reasons, this variant has been classified as Pathogenic. -

The arginine residue is moderately conserved and there is a small physicochemical difference between arginine and glutamine. This variant is not present in population databases (ExAC no frequency). This variant has been observed to be de novo in an individual affected with Sotos syndrome (PMID: 12464997), and in another individual referred for genetic testing of Sotos syndrome (PMID: 16247291). ClinVar contains an entry for this variant (Variation ID: 159395). Experimental studies have shown that this missense change has greatly reduced H3K36 histone methyltransferase activities in vitro (PMID: 21196496). The observation of one or more missense substitutions at this codon (p.Arg2005Gly) in affected individuals suggests that this may be a clinically significant residue (PMID: 26690673). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -
