Hereditary cancer-predisposing syndrome Pathogenic:1
The p.R600W variant (also known as c.1798C>T), located in coding exon 14 of the SDHA gene, results from a C to T substitution at nucleotide position 1798. The arginine at codon 600 is replaced by tryptophan, an amino acid with dissimilar properties. This variant has been observed in at least one individual with a personal and/or family history that is consistent with SDHA-related paraganglioma-pheochromocytoma syndrome (Ambry internal data). Based on internal structural analysis, the variant is moderately destabilizing to the local structure (Ambry internal data). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). Based on the majority of available evidence to date, this variant is likely to be pathogenic. -
not specified Uncertain:1
Variant summary: SDHA c.1798C>T (p.Arg600Trp) results in a non-conservative amino acid change located in the C-terminal domain (IPR015939) of the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change. The variant allele was found at a frequency of 3.1e-05 in 1595078 control chromosomes (gnomAD v4.1). This frequency is not significantly higher than estimated for a pathogenic variant in SDHA causing Neurodegeneration With Ataxia And Late-Onset Optic Atrophy, allowing no conclusion about variant significance. To our knowledge, no occurrence of c.1798C>T in individuals affected with Neurodegeneration With Ataxia And Late-Onset Optic Atrophy and no experimental evidence demonstrating its impact on protein function have been reported. ClinVar contains an entry for this variant (Variation ID: 239659). Based on the evidence outlined above, the variant was classified as uncertain significance. -
Pheochromocytoma/paraganglioma syndrome 5 Uncertain:1

not provided Uncertain:1
Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; Has not been previously published as pathogenic or benign to our knowledge -
Pheochromocytoma/paraganglioma syndrome 5;C5700310:Mitochondrial complex II deficiency, nuclear type 1 Uncertain:1
In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. The frequency data for this variant in the population databases is considered unreliable, as metrics indicate poor data quality at this position in the gnomAD database. This variant has not been reported in the literature in individuals affected with SDHA-related conditions. ClinVar contains an entry for this variant (Variation ID: 239659). Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive. This sequence change replaces arginine, which is basic and polar, with tryptophan, which is neutral and slightly polar, at codon 600 of the SDHA protein (p.Arg600Trp). -
