Genes affected
ARSB (HGNC:714): (arylsulfatase B) Arylsulfatase B encoded by this gene belongs to the sulfatase family. The arylsulfatase B homodimer hydrolyzes sulfate groups of N-Acetyl-D-galactosamine, chondriotin sulfate, and dermatan sulfate. The protein is targeted to the lysozyme. Mucopolysaccharidosis type VI is an autosomal recessive lysosomal storage disorder resulting from a deficiency of arylsulfatase B. Two alternatively spliced transcript variants encoding distinct isoforms have been found for this gene. [provided by RefSeq, Dec 2016]

Likely pathogenic, criteria provided, single submitterclinical testingLabcorp Genetics (formerly Invitae), LabcorpOct 07, 2022This sequence change replaces proline, which is neutral and non-polar, with leucine, which is neutral and non-polar, at codon 481 of the ARSB protein (p.Pro481Leu). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. Experimental studies have shown that this missense change affects ARSB function (PMID: 15979036). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt ARSB protein function. ClinVar contains an entry for this variant (Variation ID: 559713). This missense change has been observed in individual(s) with mucopolysaccharidosis type VI (PMID: 15979036). This variant is not present in population databases (gnomAD no frequency). -

Uncertain significance, criteria provided, single submitterclinical testingWomen's Health and Genetics/Laboratory Corporation of America, LabCorpSep 30, 2024Variant summary: ARSB c.1442C>T (p.Pro481Leu) results in a non-conservative amino acid change in the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 251428 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.1442C>T has been reported in the literature in a compound heterozygous individual affected with halitosis, slight photophobia, dyslexia, and mild hearing loss but not with typical presentations of Mucopolysaccharidosis Type VI (Maroteaux-Lamy Syndrome) (Brooks_2005). These report(s) do not provide unequivocal conclusions about association of the variant with Mucopolysaccharidosis Type VI (Maroteaux-Lamy Syndrome). At least one publication reports experimental evidence evaluating an impact on protein function. These results showed comparable specific activity of the mutant protein compared to that of wild type protein (Brooks_2005). The following publication have been ascertained in the context of this evaluation (PMID: 15979036). ClinVar contains an entry for this variant (Variation ID: 559713). Based on the evidence outlined above, the variant was classified as uncertain significance. -
