The p.R1521H variant (also known as c.4562G>A), located in coding exon 32 of the LAMA4 gene, results from a G to A substitution at nucleotide position 4562. The arginine at codon 1521 is replaced by histidine, an amino acid with highly similar properties. This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. The evidence for this gene-disease relationship is limited; therefore, the clinical significance of this alteration is unclear.

Variant summary: LAMA4 c.4583G>A (p.Arg1528His) results in a non-conservative amino acid change located in the Laminin G domain (IPR001791) of the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4.8e-05 in 250836 control chromosomes. The observed variant frequency is approximately 1.91 fold of the estimated maximal expected allele frequency for a pathogenic variant in LAMA4 causing Cardiomyopathy phenotype (2.5e-05). c.4583G>A has been reported in the literature in an individual affected with Cardiomyopathy (Haskell_2017). These report(s) do not provide unequivocal conclusions about association of the variant with Cardiomyopathy. Co-occurrences with other pathogenic variant(s) have been reported (DSG2 c.676_680del, p.Thr226fs), providing supporting evidence for a benign role. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication have been ascertained in the context of this evaluation (PMID: 28611029). ClinVar contains an entry for this variant (Variation ID: 213607).
