Variant summary
Our verdict is Likely pathogenic. The variant received 6 ACMG points: 6P and 0B. PM5PP3_Strong
The NM_006208.3(ENPP1):c.323G>A(p.Cys108Tyr) variant causes a missense change. The variant allele was found at a frequency of 0.0000155 in 1,612,966 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. C108F) has been classified as Likely pathogenic.

This sequence change replaces cysteine, which is neutral and slightly polar, with tyrosine, which is neutral and polar, at codon 108 of the ENPP1 protein (p.Cys108Tyr). This variant is present in population databases (rs763922486, gnomAD 0.004%). This variant has not been reported in the literature in individuals affected with ENPP1-related conditions. Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt ENPP1 protein function with a positive predictive value of 80%. This variant disrupts the p.Cys108 amino acid residue in ENPP1. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 31826312). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
