Lafora disease Uncertain:1
The p.Pro211Leu variant in EPM2A has been reported in 2 individuals with Lafora disease (internal data, PMID: 30143794), and has been identified in 0.003% (2/74890) of African/African American chromosomes by the Genome Aggregation Database (gnomAD, http://gnomad.broadinstitute.org; dbSNP rs148475381). Although this variant has been seen in the general population in a heterozygous state, its frequency is low enough to be consistent with a recessive carrier frequency. This variant has also been reported in ClinVar (Variation ID: 576169) and has been interpreted as a variant of uncertain significance by Invitae and Ambry Genetics. Of the affected individuals, one was a compound heterozygote that carried a reported pathogenic variant in trans, which increases the likelihood that the p.Pro211Leu variant is pathogenic (Variant ID: 3098, PMID: 30143794). Computational prediction tools and conservation analyses suggest that this variant may impact the protein, though this information is not predictive enough to determine pathogenicity. The phenotype of an individual heterozygous for this variant is highly specific for Lafora disease based on Lafora bodies identified via biopsy consistent with disease (PMID: 30143794). In summary, while there is some suspicion for a pathogenic role, the clinical significance of the p.Pro211Leu variant is uncertain. The p.P211L variant (also known as c.632C>T), located in coding exon 3 of the EPM2A gene, results from a C to T substitution at nucleotide position 632. The proline at codon 211 is replaced by leucine, an amino acid with similar properties. This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

This sequence change replaces proline, which is neutral and non-polar, with leucine, which is neutral and non-polar, at codon 211 of the EPM2A protein (p.Pro211Leu). This variant is present in population databases (rs148475381, gnomAD 0.007%). This variant has not been reported in the literature in individuals affected with EPM2A-related conditions. ClinVar contains an entry for this variant (Variation ID: 576169). Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C35"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
