PKHD1 (HGNC:9016): (PKHD1 ciliary IPT domain containing fibrocystin/polyductin) The protein encoded by this gene is predicted to have a single transmembrane (TM)-spanning domain and multiple copies of an immunoglobulin-like plexin-transcription-factor domain. Alternative splicing results in two transcript variants encoding different isoforms. Other alternatively spliced transcripts have been described, but the full length sequences have not been determined. Several of these transcripts are predicted to encode truncated products which lack the TM and may be secreted. Variant summary: PKHD1 c.2269A>C (p.Ile757Leu) results in a conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be tolerated. The variant allele was found at a frequency of 1.2e-05 in 251132 control chromosomes. c.2269A>C has been reported in the literature in at least three compound heterozygous individuals (including two fetal cases) who were affected with Polycystic Kidney with- or without hepatic involvement (Rossetti_2003, Sharp_2005, Burgmaier_2021, internal data). These data indicate that the variant is likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 33940108, 12846734, 15805161).ClinVar contains an entry for this variant (Variation ID: 290977). Based on the evidence outlined above, the variant was classified as likely pathogenic. -
