Alternative splicing results in two transcript variants encoding different isoforms. Other alternatively spliced transcripts have been described, but the full length sequences have not been determined. Several of these transcripts are predicted to encode truncated products which lack the TM and may be secreted. It is reported at a frequency of 0.00017 in the Latino population of the Exome Aggregation Consortium, but this frequency is based on two alleles only in a region of good sequencing coverage. Therefore, the variant is presumed to be rare. The evidence for this variant is limited. The p.Ile757Leu variant is therefore classified as a variant of unknown significance but suspicious for pathogenicity for autosomal recessive polycystic kidney disease. This variant was observed by ICSL as part of a predisposition screen in an ostensibly healthy population. -

Five of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 1.2e-05 in 251132 control chromosomes (gnomAD). The variant, c.2269A>C, has been reported in the literature in at least three compound heterozygous individuals (including two fetal cases) who were affected with Polycystic Kidney with- or without hepatic involvement (Rossetti_2003, Sharp_2005, Burgmaier_2021).These data indicate that the variant may be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Six clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. Based on the evidence outlined above, the variant was classified as VUS-possibly pathogenic. -
