Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt CFTR protein function with a positive predictive value of 80%. This variant disrupts the p.Gly126 amino acid residue in CFTR. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 23974870). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. The p.G126A variant (also known as c.377G>C), located in coding exon 4 of the CFTR gene, results from a G to C substitution at nucleotide position 377. The glycine at codon 126 is replaced by alanine, an amino acid with similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on internal structural analysis, this variant does not decrease structural stability (Liu F et al. Cell, 2017 03;169:85-95.e8). Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Variant summary: CFTR c.377G>C (p.Gly126Ala) results in a non-conservative amino acid change located in the ABC transporter type 1, transmembrane domain (IPR011527) of the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 250898 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.377G>C in individuals affected with Cystic Fibrosis and no experimental evidence demonstrating its impact on protein function have been reported.
