The p.Q493L variant (also known as c.1478A>T), located in coding exon 11 of the CFTR gene, results from an A to T substitution at nucleotide position 1478. The glutamine at codon 493 is replaced by leucine, an amino acid with dissimilar properties. This alteration was identified in a three year old with chronic cough, vomiting, pancreatic insufficiency and a sweat chloride of 92 mEq/L (Sharma N et al. Ann Hum Genet, 2009 Jan;73:26-33). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
This variant was identified in 1 patient with a clinically confirmed diagnosis of cystic fibrosis. The variant was classified in the context of a project re-classifying variants in the German Cystic Fibrosis Registry (Muko.e.V.).
