Variant summary
Our verdict is Pathogenic. The variant received 12 ACMG points: 12P and 0B. PVS1PM2PP5_Moderate
The NM_000083.3(CLCN1):c.450C>A(p.Tyr150*) variant causes a stop gained change involving the alteration of a non-conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Pathogenic (★). Synonymous variant affecting the same amino acid position (i.e. Y150Y) has been classified as Likely benign. Variant results in nonsense mediated mRNA decay.

Congenital myotonia, autosomal recessive form;C2936781:Congenital myotonia, autosomal dominant form Pathogenic:1
This variant is not present in population databases (gnomAD no frequency). For these reasons, this variant has been classified as Pathogenic. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. This premature translational stop signal has been observed in individual(s) with clinical features of myotonia congenita (PMID: 29480456). It has also been observed to segregate with disease in related individuals. This sequence change creates a premature translational stop signal (p.Tyr150*) in the CLCN1 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in CLCN1 are known to be pathogenic (PMID: 17932099, 22094069, 23739125).
Congenital myotonia, autosomal recessive form Pathogenic:1
The variant is not observed in the gnomAD v4.0.0 dataset. Predicted Consequence/Location: Stop-gained (nonsense): predicted to result in a loss or disruption of normal protein function through nonsense-mediated decay (NMD) or protein truncation. Multiple pathogenic variants are reported downstream of the variant. The variant has been reported to be associated with CLCN1-related disorder (ClinVar ID: VCV002927187 /PMID: 29480456). Therefore, this variant is classified as Pathogenic according to the recommendation of ACMG/AMP guideline.
