The frequency data for this variant in the population databases is considered unreliable, as metrics indicate poor data quality at this position in the gnomAD database. This missense change has been observed in individual(s) with long QT syndrome (PMID: 18441445). ClinVar contains an entry for this variant (Variation ID: 67495). This missense variant replaces alanine with threonine at codon 1144 of the KCNH2 protein. Computational prediction is inconclusive regarding the impact of this variant on protein structure and function (internally defined REVEL score threshold 0.5 < inconclusive < 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in an individual affected with long QT syndrome (PMID: 18441445), in another suspected of having epilepsy, in one affected with cardiovascular disease, and in two individuals with other non-cardiovascular diseases (PMID: 31696929). This variant has been identified in 2/219864 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.

The p.A1144T variant (also known as c.3430G>A), located in coding exon 15 of the KCNH2 gene, results from a G to A substitution at nucleotide position 3430. The alanine at codon 1144 is replaced by threonine, an amino acid with similar properties. This alteration has been reported in a long QT syndrome genetic testing cohort; however, clinical details were limited (Nagaoka I et al. Circ J, 2008 May;72:694-9). This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
