Four copies of this protein interact with one copy of the KCNE2 protein to form a functional potassium channel. Mutations in this gene can cause long QT syndrome type 2 (LQT2). The arginine at codon 887 is replaced by cysteine, an amino acid with highly dissimilar properties, and is located in the C-terminal region of the protein. An alternate amino acid substitution at this codon, p.R887H, was described in a long QT syndrome genetic testing cohort and in a primary electrical syndrome cohort (Tester et al. Heart Rhythm. 2005;2(5):507-17; Adler A et al. Circ Arrhythm Electrophysiol. 2016;9:e003440). Based on data from gnomAD, the frequency for this variant is above the maximum credible frequency for a disease-causing variant in this gene based on internally established thresholds (Karczewski et al. Nature. 2020 May;581(7809):434-443; Whiffin et al. Genet Med. 2017 10;19:1151-1158). This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis.
