Variant summary

Our verdict is Pathogenic. The variant received 18 ACMG points: 18P and 0B. PVS1PM2PP5_Very_Strong

The NM_000238.4(KCNH2):c.1956T>A(p.Tyr652*) variant causes a stop gained change involving the alteration of a non-conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Pathogenic (★★). Synonymous variant affecting the same amino acid position (i.e. Y652Y) has been classified as Benign. Variant results in nonsense mediated mRNA decay.

Genes affected
KCNH2 (HGNC:6251): (potassium voltage-gated channel subfamily H member 2) This gene encodes a component of a voltage-activated potassium channel found in cardiac muscle, nerve cells, and microglia. Four copies of this protein interact with one copy of the KCNE2 protein to form a functional potassium channel. Mutations in this gene can cause long QT syndrome type 2 (LQT2). Transcript variants encoding distinct isoforms have been identified. [provided by RefSeq, May 2022]

ACMG classification

Classification was made for transcript

Our verdict: Pathogenic. The variant received 18 ACMG points.

PVS1
Loss of function variant, product undergoes nonsense mediated mRNA decay. LoF is a known mechanism of disease.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 7-150951110-A-T is Pathogenic according to our data. Variant chr7-150951110-A-T is described in ClinVar as Pathogenic. ClinVar VariationId is 200402.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.

p.Tyr652Stop (TAT>TAA): c.1956 T>A in exon 8 of the KCNH2 gene (NM_000238.2). The Tyr652Stop mutation in the KCNH2 gene has been reported in one family with LQTS (Sun Y et al., 2009). Tyr652Stop is predicted to cause loss of normal protein function either by protein truncation or nonsense-mediated mRNA decay. Other nonsense mutations in the KCNH2 gene have been reported in association with LQTS. In summary, Tyr652Stop in the KCNH2 gene is interpreted as a disease-causing mutation. The variant is found in LQT panel(s). -

This sequence change creates a premature translational stop signal (p.Tyr652*) in the KCNH2 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in KCNH2 are known to be pathogenic (PMID: 10973849, 19862833). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with KCNH2-related conditions. ClinVar contains an entry for this variant (Variation ID: 200402). For these reasons, this variant has been classified as Pathogenic. -
