Four copies of this protein interact with one copy of the KCNE2 protein to form a functional potassium channel. Mutations in this gene can cause long QT syndrome type 2 (LQT2). This variant has been observed in individual(s) with clinical features of KCNH2-related conditions (Invitae). Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Tolerated"; PolyPhen-2: "Possibly Damaging"; Align-GVGD: "Class C0"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. This variant is not present in population databases (ExAC no frequency). This sequence change replaces isoleucine with leucine at codon 583 of the KCNH2 protein (p.Ile583Leu). The isoleucine residue is highly conserved and there is a small physicochemical difference between isoleucine and leucine.

The p.I583L variant (also known as c.1747A>C), located in coding exon 7 of the KCNH2 gene, results from an A to C substitution at nucleotide position 1747. The isoleucine at codon 583 is replaced by leucine, an amino acid with highly similar properties, and is located in the S5/pore region of the protein. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
