Variant summary
Our verdict is Uncertain significance. Variant got 5 ACMG points: 9P and 4B. PM1PM5PP3_StrongPP5BS2
The NM_000238.4(KCNH2):c.1283C>T(p.Ser428Leu) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.0000062 in 1,614,070 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. S428P) has been classified as Likely pathogenic.

Long QT syndrome Pathogenic:1
Likely pathogenic, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Oct 25, 2023 | This sequence change replaces serine, which is neutral and polar, with leucine, which is neutral and non-polar, at codon 428 of the KCNH2 protein (p.Ser428Leu). This variant is present in population databases (rs199472899, gnomAD 0.006%). This missense change has been observed in individuals with clinical features of long QT syndrome (PMID: 11854117, 28438721; Invitae). ClinVar contains an entry for this variant (Variation ID: 67180). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Experimental studies have shown that this missense change affects KCNH2 function (PMID: 28280240). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. - |
not specified Uncertain:1
Uncertain significance, criteria provided, single submitter | clinical testing | Women's Health and Genetics/Laboratory Corporation of America, LabCorp | Feb 02, 2024 | Variant summary: KCNH2 c.1283C>T (p.Ser428Leu) results in a non-conservative amino acid change located in the Ion transport domain (IPR005821) of the encoded protein sequence. Three of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 1.6e-05 in 251480 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.1283C>T has been reported in the literature in individuals affected with Long QT Syndrome (Moss_2002, Kirchhof_2003, Napolitano_2005, Imboden_2006, Itoh_2010, Christiansen_2014, Samol_2016, Marstrand_2021, Aizawa_2023). These reports do not provide unequivocal conclusions about association of the variant with Arrhythmia. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 36861347, 24606995, 17192539, 20541041, 14521653, 34531279, 11854117, 16414944, 27379800). ClinVar contains an entry for this variant (Variation ID: 67180). Based on the evidence outlined above, the variant was classified as uncertain significance. - |
not provided Uncertain:1
Uncertain significance, criteria provided, single submitter | clinical testing | GeneDx | Jun 03, 2022 | Reported in association with LQTS; however, detailed clinical information was not provided (Moss et al., 2002); Published functional studies demonstrate that this variant leads to a reduction in membrane expression of the channel protein (Phan et al., 2017); however additional studies are needed to validate the functional effect of this variant in vivo; Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; This variant is associated with the following publications: (PMID: 28438721, 16414944, 19862833, 28476926, 11854117, 28280240) - |
