Furthermore, the R100Q mutation was not observed inapproximately 6,200 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. R100Q results in a semi-conservative amino acid substitution of Glutamine at a position that is conserved across species. Mutations at the same residue (R100G, R100W) and in nearby residues (E90K, V94G, V94M, E95G, I96T, I96V, F98S, Y99S, K101E, D102A, D102V, F106L, F106Y, C108R, L109R) have been reported in association with LQTS, further supporting the functional importance of this region of the protein. In summary, R100Q in the KCNH2 gene is interpreted as a disease-causing mutation. The variant is found in LQT panel(s). -

This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical features of long QT syndrome and/or long QT syndrome (PMID: 15840476, 31737537; Invitae). ClinVar contains an entry for this variant (Variation ID: 67460). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be tolerated. Experimental studies have shown that this missense change affects KCNH2 function (PMID: 25417810, 32475984). This variant disrupts the p.Arg100 amino acid residue in KCNH2. Other variant(s) that disrupt this residue have been observed in individuals with KCNH2-related conditions (PMID: 16922724), which suggests that this may be a clinically significant amino acid residue. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
