Variant summary: Variant affects a non-conserved nucleotide and results in a replacement of an Aspartate (D) with a Glutamic acid (E). Both residues are medium size and acidic, therefore this Aspartate to Glutamic acid substitution likely does not alter the physico-chemical properties of the protein. It is absent from the large and broad cohorts of the ExAC project and to our knowledge, it was not reported in Lynch syndrome spectrum patients either. In vitro/vivo studies to reporting the impact on the function of the protein were not published at the time of scoring. Due to the lack of clinical and functional data, the variant was classified as a variant of uncertain significance until more information becomes available.

The p.D286E variant (also known as c.858C>A), located in coding exon 8 of the PMS2 gene, results from a C to A substitution at nucleotide position 858. The aspartic acid at codon 286 is replaced by glutamic acid, an amino acid with highly similar properties. This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear.

This sequence change replaces aspartic acid, which is acidic and polar, with glutamic acid, which is acidic and polar, at codon 286 of the PMS2 protein (p.Asp286Glu). This variant is present in population databases (rs201921616, gnomAD 0.0009%). This variant has not been reported in the literature in individuals affected with PMS2-related conditions. ClinVar contains an entry for this variant (Variation ID: 41720). Invitae Evidence Modeling incorporating data from in vitro experimental studies (internal data) indicates that this missense variant is not expected to disrupt PMS2 function with a negative predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
