The PMS2 c.608C>G (p.Thr203Ser) variant has been detected in the published literature in individuals with breast cancer (PMIDs: 33471991 (2021) and 35884425 (2022), see also LOVD (http://databases.lovd.nl/shared)), in individuals with uterine corpus endometrial carcinoma (PMIDs: 26689913 (2015), 30311369 (2018), and 29684080 (2018)), and in reportedly healthy individuals (PMID: 33471991 (2021), see also LOVD (http://databases.lovd.nl/shared)). The variant has also been reported in along with a PMS2 copy number variant in an individual undergoing genetic testing (PMID: 29895855 (2019)). The frequency of this variant in the general population, 0.000008 (2/251494 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. Based on the available information, we are unable to determine the clinical significance of this variant.

Hereditary cancer-predisposing syndrome Uncertain:2
The p.T203S variant (also known as c.608C>G), located in coding exon 6 of the PMS2 gene, results from a C to G substitution at nucleotide position 608. The threonine at codon 203 is replaced by serine, an amino acid with similar properties. In a study of whole-exome sequencing in patients with features of Cowden syndrome (CS) or Bannayan-Riley-Ruvalcaba syndrome (BRRS) and negative PTEN testing, this alteration was identified in 0/87 patients with CS or BRRS and 1/3476 patients from The Cancer Genome Atlas (TCGA) (Yehia L et al. PLoS Genet, 2018 04;14:e1007352). This amino acid position is not well conserved in available vertebrate species, and serine is the reference amino acid in several species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
