Genes affected
GUSB (HGNC:4696): (glucuronidase beta) This gene encodes a hydrolase that degrades glycosaminoglycans, including heparan sulfate, dermatan sulfate, and chondroitin-4,6-sulfate. The enzyme forms a homotetramer that is localized to the lysosome. Mutations in this gene result in mucopolysaccharidosis type VII. Alternative splicing results in multiple transcript variants. There are many pseudogenes of this locus in the human genome.[provided by RefSeq, May 2014]

Our verdict: Likely_benign. The variant received -4 ACMG points.

PM1
In a hotspot region, there are 2 aminoacids with missense pathogenic changes in the window of +-8 aminoacids around while only 0 benign, 4 uncertain in NM_000181.4
BP4
Computational evidence support a benign effect (MetaRNN=0.29263932).
BS1
Variant frequency is greater than expected in population nfe. GnomAd4 allele frequency = 0.00126 (192/152218) while in subpopulation NFE AF = 0.00218 (148/67996). AF 95% confidence interval is 0.00189. There are 0 homozygotes in GnomAd4. There are 93 alleles in the male GnomAd4 subpopulation. Median coverage is 33. This position passed quality control check. Existence of Clinvar submissions makes me limit the strength of this signal to Supporting

This variant is a known pseudodeficiency allele (PMID: 7573038, 19224584). Individuals with this variant can exhibit low beta-glucuronidase activity during enzyme analysis; however, it has been observed as homozygous in population databases and in individuals who did not have clinical evidence of muccopolysacchariduria (PMID: 7573038, 9490302). For these reasons, this variant has been classified as Benign (Pseudodeficiency allele). Pseudodeficiency allele

This variant was observed as part of a predisposition screen in an ostensibly healthy population. A literature search was performed for the gene, cDNA change, and amino acid change (where applicable). Publications were found based on this search. However, the evidence from the literature, in combination with allele frequency data from public databases where available, was not sufficient to rule this variant in or out of causing disease. Therefore, this variant is classified as a variant of unknown significance. -
