Variant summary
Our verdict is Likely pathogenic. The variant received 8 ACMG points: 8P and 0B. PM1PM2PP3_Strong
The NM_001308093.3(GATA4):c.842C>T(p.Thr281Met) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.00000186 in 1,613,784 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. 14/22 in silico tools predict a damaging outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. T281S) has been classified as Uncertain significance.

This sequence change replaces threonine, which is neutral and polar, with methionine, which is neutral and non-polar, at codon 280 of the GATA4 protein (p.Thr280Met). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with congenital heart disease (PMID: 20659440). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 30108). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt GATA4 protein function with a positive predictive value of 80%. Experimental studies have shown that this missense change affects GATA4 function (PMID: 23626780). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
