Variant summary

Our verdict is Uncertain significance. The variant received 5 ACMG points: 5P and 0B. PS1_ModeratePM5PP3

The NM_001693.4(ATP6V1B2):c.1516_1518delCGAinsGGG(p.Arg506Gly) variant causes a missense change involving the alteration of a conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. It is difficult to determine the true allele frequency of this variant because it is of type MNV, and the frequency of such variant types in population databases may be underestimated and unreliable. No clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar. Another nucleotide change resulting in the same amino acid substitution has been previously reported as Likely pathogenic in ClinVar. Another variant affecting the same amino acid position, but resulting in a different missense (i.e. R506Q) has been classified as Likely pathogenic.

Genes affected
ATP6V1B2 (HGNC:854): (ATPase H+ transporting V1 subunit B2) This gene encodes a component of vacuolar ATPase (V-ATPase), a multisubunit enzyme that mediates acidification of eukaryotic intracellular organelles. V-ATPase dependent organelle acidification is necessary for such intracellular processes as protein sorting, zymogen activation, receptor-mediated endocytosis, and synaptic vesicle proton gradient generation. V-ATPase is composed of a cytosolic V1 domain and a transmembrane V0 domain. The V1 domain consists of three A, three B, and two G subunits, as well as a C, D, E, F, and H subunit. The V1 domain contains the ATP catalytic site. The protein encoded by this gene is one of two V1 domain B subunit isoforms and is the only B isoform highly expressed in osteoclasts. ACMG classification

Classification was made for transcript

Our verdict: Uncertain_significance. The variant received 5 ACMG points.

PS1
Transcript NM_001693.4 (ATP6V1B2) is affected with MISSENSE_VARIANT having same AA change as one Pathogenic present in ClinVar.
PM5
Other missense variant is known to change same aminoacid residue: Variant chr8-20220383-G-A is described in ClinVar as Pathogenic/Likely_pathogenic. ClinVar VariationId is 3342486.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.
PP3
No computational evidence supports a deleterious effect, but strongly conserved according to phyloP
