Variant summary
Our verdict is Pathogenic. The variant received 16 ACMG points: 16P and 0B. PVS1PP5_Very_Strong
The NM_024596.5(MCPH1):c.1869_1870delAT(p.Cys624fs) variant causes a frameshift change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.0000335 in 1,613,910 control chromosomes in the GnomAD database, with no homozygous occurrence. Variant has been reported in ClinVar as Likely pathogenic (★★). Variant results in nonsense mediated mRNA decay.

Microcephaly 1, primary, autosomal recessive Pathogenic:3
not provided Pathogenic:1
Identified in trans with a second MCPH1 variant in an individual with developmental delay, intellectual disability, epilepsy, severe visual impairment, microcephaly and abnormal brain MRI. This individual also had a balanced reciprocal translocation (PMID: 33094427); Nonsense variant predicted to result in protein truncation or nonsense mediated decay in a gene for which loss of function is a known mechanism of disease; Not observed at significant frequency in large population cohorts (gnomAD); This variant is associated with the following publications: (PMID: 33094427)
Autosomal recessive primary microcephaly Pathogenic:1
Variant summary: MCPH1 c.1869_1870delAT (p.Cys624X) results in a premature termination codon, predicted to cause a truncation of the encoded protein or absence of the protein due to nonsense mediated decay, which are commonly known mechanisms for disease. The variant allele was found at a frequency of 3.2e-05 in 249512 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.1869_1870delAT has been reported in the literature in at least one individual affected with neurological abnomalities and microcephaly (e.g., Cheng_2021). To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication was ascertained in the context of this evaluation (PMID: 33094427). One submitter has reported clinical-significance assessments for this variant to ClinVar after 2014 and has classified the variant as likely pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic.
