The NBN c.2071-4A>G variant has been reported in the published literature in individuals with breast and/or ovarian cancer (PMID: 35245693 (2022), 29470806 (2018)), and in an individual with therapy-related myeloid neoplasm (PMID: 33850299 (2021)). The frequency of this variant in the general population, 0.000031 (4/129000 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using software algorithms for the prediction of the effect of nucleotide changes on splicing yielded predictions that this variant may affect proper NBN mRNA splicing. Based on the available information, we are unable to determine the clinical significance of this variant. -

The c.2071-4A>G variant is observed in 1/16,252 (0.0062%) alleles from individuals of gnomAD African background in gnomAD. The c.2071-4A>G variant is novel (not in any individuals) in 1kG. For these reasons, this variant has been classified as Uncertain Significance -
The c.2071-4A>G intronic variant results from an A to G substitution 4 nucleotides upstream from coding exon 14 in the NBN gene. This variant was observed in a study of 1010 unrelated Indian patients with breast and/or ovarian cancer (Singh J et al. Breast Cancer Res Treat, 2018 Jul;170:189-196). This nucleotide position is not well conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will result in the creation or strengthening of a novel splice acceptor site. RNA studies have demonstrated that this alteration results in an incomplete splice defect; the clinical impact of this abnormal splicing is unknown at this time (Ambry internal data). Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

The NBN c.2071-4A>G variant is predicted to interfere with splicing. This variant has been reported in several individuals with breast and/or ovarian cancer (Table S4, Singh et al. 2018. PubMed ID: 29470806; Table 2, Castillo-Guardiola et al. 2022. PubMed ID: 35245693; Table 3, Rosado-Jiménez et al. 2023. PubMed ID: 38075165). This variant is reported in 0.0040% of alleles in individuals of African descent in gnomAD. This variant is predicted to alter splicing based on available splicing prediction programs (SpliceAI, Jaganathan K, et al. 2019. PubMed ID: 3066175). However, the use of computer prediction programs is not equivalent to functional evidence. This variant is interpreted as a variant of uncertain significance in ClinVar (https://www.ncbi.nlm.nih.gov/clinvar/variation/219601/). At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence. -
