The encoded protein is a member of the MRE11/RAD50 double-strand break repair complex which consists of 5 proteins. The NBN c.2070+4G>A intronic change results in an G to A substitution at the +4 position of intron 13 of the NBN gene. Algorithms that predict the impact of sequence changes on splicing indicate that this variant does not affect splicing, and this has been confirmed by RNA studies (internal data). This variant has a maximum subpopulation frequency of 0.012% in gnomAD v2.1.1 (https://gnomad.broadinstitute.org/). To our knowledge, this variant has not been reported in individuals with Nijmegan breakage syndrome. In summary, this variant meets criteria to be classified as likely benign. -

In silico splice site analysis predicts that this alteration will not have any significant effect on splicing. The frequency of this variant in the general population, 0.00012 (4/34508 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using software algorithms for the prediction of the effect of nucleotide changes on splicing yielded predictions that this variant does not affect NBN mRNA splicing. Variant summary: NBN c.2070+4G>A alters a non-conserved nucleotide located close to a canonical splice site and therefore could affect mRNA splicing, leading to a significantly altered protein sequence. 4/4 computational tools predict no significant impact on normal splicing. However, these predictions have yet to be confirmed by functional studies. The variant allele was found at a frequency of 1.6e-05 in 250020 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.2070+4G>A has been reported in the literature as a VUS in a setting of multigene panel testing in an individual affected with breast cancer (Tung_2015). However, this report does not provide information about association of the variant with Nijmegen Breakage Syndrome. These report(s) do not provide unequivocal conclusions about association of the variant with Nijmegen Breakage Syndrome. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Six other clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation and classified the variant as VUS (n=5) or likely benign (n=1). Based on the evidence outlined above, the variant was classified as VUS-possibly benign. -
