The encoded protein is a member of the MRE11/RAD50 double-strand break repair complex which consists of 5 proteins. This variant was absent in 121360 control chromosomes and has not, to our knowledge, been reported in affected individuals via publications, nor evaluated for functional impact by in vivo/vitro studies. In addition, one clinical diagnostic laboratory classified this variant as a VUS. Because of the absence of clinical information and the lack of functional studies, the variant was classified as a variant of uncertain significance (VUS) until additional information becomes available. -

The NBN c.1370A>G (p.Asn457Ser) variant has been reported in an individual with breast cancer in a published case-control study (PMID: 33471991 (2021), see also LOVD (http://databases.lovd.nl/shared/genes/NBN)). In addition, this variant has also been reported in large pan-cancer screening study as a variant of uncertain significance (PMID: 36346689 (2023)). This variant has not been reported in large, multi-ethnic general populations (Genome Aggregation Database, http://gnomad.broadinstitute.org). Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. Based on the available information, we are unable to determine the clinical significance of this variant. -

This variant is denoted NBN c.1370A>G at the cDNA level, p.Asn457Ser (N457S) at the protein level, and results in the change of an Asparagine to a Serine (AAC>AGC). This variant has not, to our knowledge, been published in the literature as pathogenic or benign. NBN Asn457Ser was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. Since Asparagine and Serine share similar properties, this is considered a conservative amino acid substitution. NBN Asn457Ser occurs at a position that is conserved across species and is not located in a known functional domain (UniProt). In silico analyses are inconsistent regarding the effect this variant may have on protein structure and function. Based on currently available information, it is unclear whether NBN Asn457Ser is pathogenic or benign. We consider it to be a variant of uncertain significance. -

The p.N457S variant (also known as c.1370A>G), located in coding exon 10 of the NBN gene, results from an A to G substitution at nucleotide position 1370. The asparagine at codon 457 is replaced by serine, an amino acid with highly similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
